As we gear up for Healthier SG, we have begun to shift our emphasis towards preventive care, where we activate our residents to own their health and support them to age in place. A key enabler of this expanded mission of Population Health is the strong partnerships of health and social care providers at the neighbourhood level, where we leverage one another's strengths to holistically meet the needs of residents.
Themed 'Caring for Our Community Together', CHALK 2022 brought Central Health partners together to share and co-learn good practices in building Communities of Care (CoCs), explore collaborations and collectively craft how we can continue to care for our community together.
Our first hybrid CHALK event was attended both online and onsite by close to 300 participants from 41 organisations. We saw many collaborative initiatives featured in this year's poster competition. Participants also got to hear from esteemed overseas speakers (Dr Matthew Fox and his colleagues from Torbay and South Devon NHS Foundation Trust) as well as local panel members (Dr Christina Tiong from Home Nursing Foundation and Mr Andy Lee from Thye Hua Kwan Moral Charities), who generously shared their stories and experiences in building a successful network of integrated care.
Over the half day event, we had conversations on patient- and resident-centred care, volunteer management, as well as the platforms and mindset shifts required to enable multilateral collaborations amongst different care partners. During the CHALK In-Action segment, the event hall was bustling with excitement as participants broke into eight neighbourhoods and discussed what they could do together to activate their respective neighbourhoods for health.
A big thank you to our poster judges, panelists and co-facilitators from AWWA, Care Community Services Society, Care Corner Singapore, Fei Yue Community Services, Home Nursing Foundation, Kwong Wai Shiu Hospital, Methodist Welfare Services, NTUC Health, St Luke's ElderCare, Thye Hua Kwan Moral Charities, TOUCH Community Services and Tsao Foundation for making CHALK 2022 possible! We are excited to continue these conversations in the neighbourhoods and further explore other opportunities as we continue our journey to build healthier and happier communities as one Central Health.
