At Care Corner Seniors Services, we are ever mindful of quality of life in our seniors. Our team overseeing the nine Active Ageing Centres (AAC) in Toa Payoh and Woodlands interact with and engage the seniors regularly. For many of these seniors, visiting the centres has become an integral part of their routine. Over the years, we have seen their health decline. While we recognise frailty is inevitable, our experience working with seniors has also shown us that frailty is also reversible if there is early intervention. As more individuals are looking to age well within the community as opposed to long-term institutional care, we saw the need to support many more through targeted and systemic interventions that are research-based and, more importantly, beneficial to our seniors.

These was the thought process behind Stronger Together, a frailty management programme conceptualised by a small but multidisciplinary team comprising Programme Executives and Wellness Trainers from Care Corner and Physiotherapist and Dietician from TTSH. Stronger Together looks to onboard and screen seniors at risk from three dimensions of frailty: Physical, Nutritional and Psychological.

Each dimension of frailty represents a series of action plans and interventions such as resistance-based exercise to enhance muscle strength (Physical), education on protein-based diets to promote muscle growth (Nutritional) and case management support services to address motivational and social isolation issues (Psychological).

As every individual is unique, our programme is based on Person-Centred Care where care plans and health goals are carefully discussed and planned together with our seniors. By involving our seniors in the care process, it motivates them and builds their commitment to work towards their own goals and maintain their health to live their golden years to the fullest.

With more than 14,000 seniors across the seven Toa Payoh AACs, we know we need to work with others in order to reach all of them. We are looking for volunteers to befriend seniors and to run engaging exercise sessions, and need community partners and various stakeholders to reach out and engage seniors to participate in our frailty management programme. Currently, we are piloting this program at two AACs in Toa Payoh and plan to expand it to more centres in the next few years.
