​Advance Care Planning (ACP) Clinic @ CareElderly Active Ageing Centre (AAC) (Circuit Road)

​​​Given a majority of frail and single, elderly population with multiple co-morbidities living in the rental blocks of Geylang, CareElderly Active Ageing Centre (AAC) identified the need to advocate Advanced Care Planning (ACP) amongst this group of vulnerable seniors as one of its key focus areas.

CareElderly AAC and TTSH Community Health Team (CHT) collaborated to conduct an ACP advocacy session with the aim of getting seniors to think about what matters to them and to make their own choices on their care preferences. Post-session, CareElderly AAC referred seniors who expressed readiness and interest in actualising ACP to TTSH CHT. TTSH CHT subsequently conducted a basic geriatric assessment to pick-up medico-socio related issues for more holistic exploration of personal values, beliefs and care goals, and documentation of healthcare preferences. To encourage the completion of ACP, CareElderly AAC also offered seniors an “afterlife” photography session. Till date, a total of 35 seniors have actualised their ACP.
