Social Work Case Management
Social work case management involves assessing, planning, coordinating, and advocating for individuals in need of social services. Our Medical Social Workers collaborate with patients and family members to identify goals, access resources and navigate systems.
We provide support, guidance, and referrals to address various challenges, such as housing, healthcare, and mental health. We strive to empower patient through the adoption of a holistic approach.
Counselling & Therapy
Our Medical Social Workers are trained to provide counselling and psycho-emotional support to patients and families/caregivers who face challenges in coping with illnesses and disabilities.
Risk Assessment & Crisis Intervention
Our Medical Social Workers are trained to provide risk assessment, interventions, safety planning, emotional support and psychological first-aid for patients and families with issues relating to:
- Suicide;
- Neglect and/or Interpersonal Violence; and/or
- Post-trauma
Therapeutic Group Work & Support Group
Coping with a major or chronic medical condition can be a challenge for patients and family members/caregivers. Therapeutic Group Work & Support groups are an important source of psycho-social and emotional support. These groups provide a platform for patients and family members/caregivers to share experiences and learn how to cope with their conditions better.
