Overview

The Department of General Medicine is Tan Tock Seng Hospital's largest department providing inpatient care at TTSH. We provide comprehensive and holistic care for patients with a range of simple to complex medical conditions, and coordinate diagnostic work-up of undifferentiated symptoms and management of multi-system illnesses. We work closely with other Specialists, Nursing and Allied Healthcare workers to provide timely care and interventions for our patients.

In addition to our inpatient services for acutely ill patients, our department runs the Acute Internal Medicine Service (AIMS) for short stays, Mobile Inpatient Care At Home (TTSH@Home), and subacute care for patients who need further rehabilitation.

We also actively provide inpatient consult and outpatient clinic services in the areas of General Medicine, Vascular Medicine, Peri-operative Medicine, Hypertension and Obstetric Medicine.

List of Services

The Acute Internal Medicine Service (AIMS) provides front-loaded, short-stay care to patients, with rapid access to investigations and management. We work closely with dedicated, purposefully-trained Nursing and Allied health teams, and together facilitate earlier discharges for our patients, while ensuring clinical safety coupled with efficiency.

AIMS is poised to expand its roles and capacity as we look forward to refining and championing the concept of acute care in Tan Tock Seng Hospital.

The national Mobile Inpatient Care at Home project was launched with the goal of making it possible and preferable to deliver inpatient care in the comfort of a patient's own home. The Tan Tock Seng at Home (TTSH@Home) service was launched in July 2023.

The TTSH@Home team will rigorously screen and select suitable patients who would benefit from this service, and plan their management and the logistics required to care for them at home. Many patients have expressed satisfaction at being able to continue living their lives as close to normality as possible, while welcoming the daily telemedicine consultations and the house calls from our team as being much more convenient.

We are continuing to increase our capacity to meet patients’ needs and the healthcare demands of the nation as we embrace the future.

Our Hypertension clinic specialises in investigation and management of high blood pressure, in particular, evaluation of secondary causes of hypertension as well as management of difficult to control blood pressure across all age-groups.

We also have a pharmacist-led Blood Pressure Service, started in 2022, which plays a supportive role by expediting blood pressure control through home blood pressure monitoring and tele-health management.

Our General Medicine Complex Care (GMCC) clinic manages patients with multiple complex medical conditions, who would benefit from comprehensive oversight under one primary physician. In partnership with the GMCC clinic, we also have a pharmacist-led medication review clinic which supports our patients by reviewing their medication regimen, and monitoring their adherence and tolerance to multiple medications.

Vascular Medicine offers specialized and dedicated care for patients with conditions affecting the arteries, veins, and lymphatics.

Our inpatient and outpatient service offers expertise on prophylaxis and management of venous thromboembolism (VTE), deep vein thrombosis (DVT) and pulmonary embolism (PE), chronic venous insufficiency, lymphedema and peripheral arterial disease. We extend these services to KK Women’s and Children’s hospital as well.

Our Anticoagulation Clinic (ACC) is run by pharmacists and offers counselling and monitoring for oral and parenteral anticoagulation, including peri-procedural management of anticoagulation.

The Perioperative team within General Medicine runs daily clinics at Clinic 5A and the PACE (Preoperative Counselling and Evaluation) Clinic in B2B. Working in close conjunction with Anesthesia and General Surgery, we optimize medical conditions and medications for patients prior to planned elective procedures, so as to achieve the best possible outcomes.

Obstetric Medicine is a specialized field of General Medicine that provides comprehensive care to women with medical conditions before and during pregnancy, as well as in the immediate postpartum period. With women conceiving at an older age, and co-morbidities such as hypertension, diabetes, autoimmune disorders, asthma, epilepsy, etc - Obstetric Medicine is an important facet of care to reduce pregnancy-related morbidity and mortality.

We respond to inpatient referrals for pregnant patients with medical conditions at KK Women’s and Children’s Hospital (KKH). We also run regular outpatient clinics for pre-pregnancy counselling, antenatal and postnatal obstetric medical care at KKH, and provide training to Internal Medicine and Obstetrics and Gynecology trainees.

Advance Care Planning (ACP) is the process of planning for your healthcare and personal care needs. It allows our patients to be cared for holistically, and encourages person centered care through good quality conversations and communication. Should you fall too ill to communicate or make treatment decisions, your Advance Care Plan will help your medical team and spokesperson to make decisions that are in your best interests.

The Department of General Medicine hopes to empower our patients to “Live well & Leave well” with Advance Care Planning. Should you be interested in making an Advance Care plan, please speak to your healthcare team about it.
