Request Medical Report

Requesting a medical report? Find out here on the procedures needed to request for one.​

What is required during the request for medical information?

  1. Patient's written Consent for Release of Medical Information (Form A) (PDF, 590 kb) to allow the hospital to release medical information for a stated purpose.
    • For patients below 21 years of age, consent of the parent or legal guardian is required.
    • ​For deceased or mentally incompetent patients, consent from the appointed representative of the estate is required. Where applicable, the Consent and Declaration for Release Of Medical Information for patient with Mentally Incapacitated or Deceased Patient (Form B) (PDF, 570 kb)  must be filled up. A copy of the death certificate and other supporting documents (e.g. marriage certificate, birth certificate, letters of administration) are required as proof of the representative's relationship to the patient.
    • Interested third parties can write in together with patient's consent, permitting​​ the hospital to release medical information for a stated purpose.
    • For foreign workers that have returned to their native country, employers can apply for these foreign workers' medical information by completing a Letter of Indemnity and also providing​ document proof of cancellation of work permit for the foreign worker.
  2. A copy of the patient's NRIC / identification documents.
  3. A payment for the information required. See "What is available and how much?" section for more details.
  4. Payment can be made by credit card, interbank giro or PayNow for requests via email or post.
  5. For Paynow payment, please transfer to Unique Entity No (UEN): 198702955E33M​ and indicate under payment reference number as "MRO" followed by the last 3 digits and alphabet of patient's NRIC/FIN/HRN no (e.g MRO123A).

How to request for medical information?

Requests for medical information can be made in the following manner:

    1. For the requests for patient below 21 years old and patient lack of mental capacity, please apply reports via email / walk in to MRO
    2. You may only apply for the mentioned medical report types below:
      • MRRP - Ordinary Medical Report: This is a factual report of the patient’s medical condition. A doctor will write the medical report based on the patient’s medical information available. (Insurance form or type-written report.)
      • MRRP - Specialist Medical Report: This report is prepared by the patient’s specialist in response to request that require a professional opinion with regards to the patient’s specialist in response to requests that require a professional opinion with regards to the patient’s prognosis and disabilities. It is based on an actual assessment of the patient and may involve a review at the Specialist Outpatient Clinic (Insurance for or type-written report.)
      • MRRP - Duplicate Document (MC / Medical Report): A duplicate copy of the medical report that was previously applied before or a reprint of medical certificate previously issued.
      • MRRP - Investigation results: Selective duplicate copies of investigation results such as blood test results, lab results, investigation reports (ECG, Electrophysiology, EMGS), X-ray and other radiological reports can be made available.

Ordinary Medical Report 

This is a factual report of the patient's medical condition. A doctor will write the medical report based on patient's medical information available. (Insurance form or type-written report)

Work Injury Compensation (WIC) Initial Assessment Report 

This is for the purpose of determining work-related injuries and the degree and period of disability results of a work injury. Requester to provide MOM's WIC initial assessment form or MOM request letter for the application. May involve a review at the Specialist Outpatient Clinic.

Specialist Medical Report  

This report is prepared by the patient's specialist in response to requests that require a professional opinion with regards to the patient's prognosis and disabilities. It is based on an actual assessment of the patient and may involve a review at the Specialist Outpatient Clinic. (Insurance form or type-written report)

Work Injury Compensation Objection Assessment Report

This is for the purpose of re-determining work-related injuries and the degree and period of disability when patient objects to the results of the WIC initial assessment report raised. The WIC objection report is done by two doctors who are consultants and above. We accept WIC objection report request for patients who are assessed by doctors of other hospital for the initial assessment report.

Specialist Medical Report (Simple - Psychiatric)  

This report is prepared by the assigned psychiatrist in response to requests that require a professional opinion with regards to the patient’s assessment or condition. The requirement for further assessment for purpose of preparing the report is subject to review by the assigned/ treating psychiatrist in the Specialist Outpatient Clinic.

Specialist Medical Report (Complex - Psychiatric)

This report is prepared by the assigned psychiatrist in response to requests that require detailed professional opinions not covered by a simple report, with regards to the patient’s assessment or condition, which may extend to domains of prognosis and disability. The requirement for further assessment for purpose of preparing the report is subject to review by the assigned/ treating psychiatrist in the Specialist Outpatient Clinic.

Medical report for Court Appointed Deputy Application (Form 224), patient has to be examined by a psychiatrist. At present, a lawyer is required to initiate the request and we are unable to provide reports for applicants acting in person/via iFAMS.

Lasting Power of Attorney Certificate Issuance 

The LPA Certificate may be issued by accredited medical practitioners, lawyers and psychiatrists. As charges vary according to complexity of the cases, members of the public are advised to check on their requirements with the Office of the Public Guardian before selecting a certificate issuer.

Ordinary/ Factual Medical Report (Psychiatric) 

This is a factual report of the patient’s medical condition based on clinical information that is already available. No assessment will be required for preparation of the report. In the event that a specific psychiatric opinion is required, the report will be considered as Specialist Medical Report (Simple – Psychiatric) or (Complex – Psychiatric).

Second Opinion Reports  

This report is written for people who are not patients of TTSH but seek a professional opinion from our Specialists. Its format and scope is the same as the Specialist Medical report. It is based on the assessment of the patient and involves a review at the Specialist Outpatient Clinic.

When can the requested medical information be ready?

As a general guide, the time frame for completion of medical reports is between 4 to 6 weeks.

It may take longer if:

  • Patient has upcoming clinic appointments
  • Patient has been hospitalised
  • Multiple reports are required from several clinical departments
  • Doctor is away

Specialist Medical Reports and Workmen Compensation cases require more processing time as a review at the Specialist Outpatient Clinic may be required after the patient has been discharged or given open date from clinic review.​

How can I receive the medical report/insurance form/copies of medical records?

Patients can request for the medical report to be:

  1. Emailed to them - Email address to be provided at the point of application.
  2. Mailed to them - Mailing address to be provided at the point of application.
  3. For self-collection at HIS, the patient's contact number is to be provided at the point of application. The following documents should be brought along for authentication purpose:
