Old age and mortality are inevitable and a part of life. The team at
Life Point advocates for autonomy in making informed decisions on ageing and end-of-life matters that aligns with one’s values and beliefs. As such, Life Point provides a one-stop service for end-of-life planning to ensure that our clients preserve their dignity and self-worth even during their final days.
A recent client of Life Point, Mary, witnessed how her close friend Rose enjoyed a dignified death despite suffering from late-stage cancer. She recounted how composed and at peace her friend was when all her wishes for non-invasive treatment and death at home were honoured and supported by her loved ones. Rose had also shared with Mary that planning for her care using Advance Care Plan (ACP) had empowered her to clearly express her values and desires for a peaceful and dignified departure. Rose then encouraged Mary to do her ACP with Life Point.
Since then, Mary has completed her ACP and expressed that she is enjoying peace of mind regarding her future health and personal care. Lasting Power of Attorney (LPA) was introduced to her and her son during the ACP session, and she was encouraged by her son to do her LPA as well.
Life Point’s services range from education, facilitation, to application and completion of important documents such as LPA, ACP, Advance Medical Directive, Will Making and Deputyship. We work with partners like the Chinese Development Assistance Council (CDAC) to conduct group workshops on end-of-life matters, and hold individual sessions for healthy and mobile seniors in the community.
We also reach out to clients who are in hospitals, nursing homes, or homebound to help them plan for their care needs and final days. In partnership with St Luke’s Nursing Home, our team works with the home’s Medical Social Workers to help families of seniors with diminished mental capacity to apply for deputyship, and assist seniors in pre-planning their final rites.
Separately, we work with commercial partners to offer pro-bono funeral services to seniors with no next-of-kin and with financial difficulties, so that they too can have a dignified departure.
