It takes a whole kampung to enable a person to maintain good health and self-care in the comfort of their homes. As our population ages and the burden of chronic diseases rises, we recognise the need for close collaborations with our health and social care partners in the community to enable neighbourhood-based care to thrive.

Launched in 2017, Centre for Health Activation (CHA) was set up to focus on Activation, Research and Training – also known as the ART of CHA. Its vision is to drive activation and build One Community of Carers (i.e. patients, caregivers, volunteers, health and social care partners) who are equipped with the skills, knowledge and confidence to self-care and care for their loved ones and others in the community.

Caring for Carers

CHA builds engagements and partnerships in Central Health via our signature community-enabling programmes – CHArge Up! Learning Programme and CHAPS Club.

CHArge Up! Learning Programme

Conducted by our TTSH healthcare professionals, CHArge Up! Learning Programme aims to equip carers with the health skills, knowledge and confidence to enable them to self-care (i.e. activation) and look out for others in the community. Find out more on events organised by CHA.

Peer Support Leaders

CHA aims to develop Peer Support Leaders (PSLs) with the skills, knowledge and confidence to lead and empower peers to achieve a healthy lifestyle in partnership with our Community Health Teams (CHTs) and Community Partners.

PSLs can be anyone – residents, patients, caregivers, volunteers, even grassroot leaders. Eventually, PSLs will be able to lead and sustain programmes and activities for their peers in their own communities.

Centre for Health Activation

 

Centre for Health Activation (CHA) is a dedicated space to engage and empower our patients, families, caregivers, community partners and healthcare professionals and connect them to healthcare resources (e.g. disease-specific brochures, wellness brochures and healthcare schemes and grants application) and platforms for learning (e.g. month​ly health talks and programmes).

Tan Tock Seng Hospital, Level 1 (between Toast Box and Lifeline)

Opening Hours:
9.30 am to 4.30 pm (Mondays to Fridays)
9.30 am to 12.30 pm (Saturdays)
