As we work together towards building a health system that focuses beyond illness to wellness, there are things already happening in your neighbourhoods to support our residents to live well and age well at home. These building blocks address some of the local health and social needs that our communities require, and allow us to bring integrated care closer to them.
One such building block is integrating health and social care to support the residents’ needs holistically.
Kwong Wai Shiu Care Centres serve as the go-to point for seniors, offering active ageing programmes, social support services and case management services. In addition, they have a Community Management Service team that proactively reaches out to seniors through door-to-door engagement to provide social support and at the same time, identify those who need health attention and link them up with the right partners.
Community Health and Wellness
In Central Singapore, we have a good network of providers that takes care of the physical and mental well-being of our residents. Let me share some good community health and wellness initiatives that promotes better health through healthy living and disease prevention.
Advocating healthy lifestyle and activating residents to take charge of their health is an important element to wellness.
FitterLife, a weight management programme by
National Healthcare Group Polyclinics is an initiative to help residents to fight obesity and lead healthier lifestyle. The group and community setting help the residents to stay positive and motivated to work towards their health goals together.
With the recent callout to get vaccinated against COVID-19,
SATA CommHealth’s Mobile Vaccination Teams have been travelling around Singapore to vaccinate the seniors and the vulnerable at their doorstep. This is to provide convenient access and protect those who are homebound.
10th October is World Mental Health Day, a day to raise mental health awareness. In this issue
TOUCH Community Services shares with us care tips to achieve mental wellness, especially for those who are experiencing a challenging time as caregivers.
