Our Kwong Wai Shiu Care (KWSC) Centres - KWSC @ McNair, KWSC @ St George and KWSC @ MacPherson, were awarded the provision of the Eldercare Centre (ECC) Model by the Ministry of Health (MOH) from February 2021. With the integration of the ECC Model, the KWSC now serves as a go-to point for seniors residing within the community for active ageing programmes, befriending, and support services such as provision of information and referral services, as well as case management services regardless of their socioeconomic background and frailty levels.
We have been supporting and reaching out to the seniors within our population-based designated service clusters in McNair (440 seniors), St George’s (491 seniors) and MacPherson (386 seniors). These service clusters will be expanded over the next two years to support seniors within the central region (e.g. at least 7,000 seniors within the McNair and St George’s areas).
Partnering with MOH, the Agency for Integrated Care (AIC), Tan Tock Seng Hospital (TTSH), Health Promotion Board (HPB), SG Digital and more, we deliver a wide array of active ageing programmes and courses. These include exercise classes, digital services workshops, acrylic painting, choir sessions, outdoor excursions, Happy Exercise Programme, TTSH Community Health Post and One-to-One Digital Coaching.
In addition, Kwong Wai Shiu Hospital (KWSH) also has a Community Management Service (CMS) team who concurrently reaches out vigorously to seniors in the service clusters through door-to-door engagement, providing biopsychosocial support, financial assistance and referral to relevant services as needed. Through these efforts, we have identified seniors who are frail, isolated, or living with chronically ill spouses, and rendered befriending and buddying services cum Assisted Living Services to them respectively.
In collaboration with AIC to support seniors residing at Serangoon Road, Potong Pasir and MacPherson, we are also providing a complimentary wheelchair-friendly shuttle bus service. With this, seniors may conveniently drop by our Active Ageing Hubs for activities while those with healthcare needs may visit KWSH’s Traditional Chinese Medicine Centre and the new Kallang Polyclinic.
With the establishment of our ECC model, CMS and our shuttle bus service, seniors and their caregivers staying within the service clusters vicinity may now proactively engage in their ageing-in-place process and easily navigate our seamless and integrated network.
