It takes a whole kampung to enable a person to maintain good health and self-care in the comfort of their homes. As our population ages and the burden of chronic diseases rises, we recognise the need for close collaborations with our health and social care partners in the community to enable neighbourhood-based care to thrive.

Launched in 2017, Centre for Health Activation (CHA) was set up to focus on Activation, Research and Training – also known as the ART of CHA. Its vision is to drive activation and build One Community of Carers (i.e. patients, caregivers, volunteers, health and social care partners) who are equipped with the skills, knowledge and confidence to self-care and care for their loved ones and others in the community.

CHA builds engagements and partnerships in Central Health via our signature community-enabling programmes – CHArge Up! Learning Programme and CHAPS Club.

Conducted by our TTSH healthcare professionals, CHArge Up! Learning Programme aims to equip carers with the health skills, knowledge and
confidence to enable them to self-care (i.e. activation) and look out for others in the community.

Trained carers will be empowered to: 

  1. Recognise resident's basic health needs
  2. Identify residents that would benefit from community health programmes
  3. Highlight simple cases for health or social care intervention

Our Modules

Each module comprises of theory and skills-based assessment.
  1. Advance Care Planning (ACP) Advocacy
    • Describe what is Advance Care Planning (ACP)
    • Share what are the roles of an ACP advocate
    • Demonstrate how to broach ACP
  2. Creating a Safe Home Environment to Reduce Risks of Falls and Proper Handling of a Wheelchair
    • Identify areas within a home that pose high fall risks
    • Provide possible improvements for an elder-friendly home environment
    • Understand proper handling of a wheelchair and simple transfer techniques
    • Know resources to build an elder-friendly home environment
  3. Fostering Positive Medication Habits
    • Discuss the issues faced by older persons in managing medications
    • Develop a medication list based on medication labels
    • Pack a pillbox according to the medication list
    • Provide simple advice on issues to note on medication storage
  4. Increasing Physical Activity And Enhancing Mobility
    • Differentiate between exercise and general physical activity
    • Explain and apply the Mobility Assessment Tool (MAT)
    • Understand the correct way of using walking aids
    • Provide handhold assistance to a frail older person while walking
  5. Promoting Good Practices In Chronic Disease Management
    • Understand what are chronic diseases
    • Learn about cholesterol, hypertension and Diabetes Mellitus
    • Identify good and bad readings for cholesterol, blood pressure and blood glucose
    • Learn how to use a blood pressure machine

The Peer Support Leaders (PSL) is a membership-based programme that aims to develop one with the skills, knowledge and confidence to lead and empower peers to achieve a healthy lifestyle in partnership with our Community Health Teams (CHTs) and Community Partners.

PSLs can be anyone – residents, patients, caregivers, volunteers, even grassroot leaders. Eventually, PSLs will be able to lead and sustain programmes and activities for their peers in their own communities.

Centre for Health Activation (CHA) is a dedicated space to engage and empower our patients, families, caregivers, community partners and healthcare professionals and connect them to healthcare resources (e.g. disease-specific brochures, wellness brochures and healthcare schemes and grants application) and platforms for learning (e.g. monthly health talks and programmes).
