​​​We collaborate with General Practitioners and Polyclinics to facilitate patient access to care for acute ailments and the management of chronic conditions. Our aim is that each patient is assigned a dedicated Primary Care physician, to ensure seamless patient care integration during transition between community and hospital settings.

By ensuring every patient has a designated Primary Care physician, we foster continuity of care and enhance patient outcomes across different healthcare environments.

As part of the national Healthier SG plan (Healthier SG), we collaborate with GPs to assist residents in achieving their health goals and connect them with community resources such as programmes, activities and services.

GPFirst - Your Family Doctor, Your First Stop

The GPFirst programme encourages residents with mild to moderate, or non-emergency conditions to first seek treatment with a family doctor for more timely medical treatme​nt rather than at the acute hospital's Emergency Department (ED). This also enables ED to focus on more urgent cases requiring medical attention.

Community Right Siting Programme (CRiSP)

Community Right Siting Programme (CRiSP) is an initiative where stable patients from the Specialist Outpatient Clinics (SOCs) are discharged and followed-up by our General Practitioner (GP) partners.
