Tertiary Rehabilitation
We specialise in providing higher-intensity rehabilitation programmes to patients with complex needs like stroke, traumatic brain injury, spinal cord injury (including ventilatory rehab), orthopaedic (musculoskeletal) and trauma.
Our multidisciplinary model of care, complemented by rehabilitative technology and robotic machines, will help patients return to their daily function and subsequently to work and life as much as possible.
Intermediate Rehabilitation
Rehabilitation and therapy services will help patients following acute illnesses to regain their functional status and physical strength to perform their daily activities, such as cooking and moving around the community, which helps with reintegration back home.
Patients will be taken care by a multi-disciplinary team comprising doctors, nurses, therapists and other allied health staff, adopting a holistic approach to rehabilitation. They would be exposed to cultural and social forms of therapy including horticultural therapy.
Subacute Care
Subacute Care supports patients' recovery phase from an acute illness or injury. Subacute Care patients are clinically stable but require additional periods of multi-disciplinary medical and nursing care, as well as therapists' inputs.
As their medical care needs are attended to, Subacute Care patients will continue to receive physical, occupational, cognitive, and speech therapies, tailored according to their physical and psychosocial status.
Subacute Care patients in the geriatric age-group with dementia-related complications or who are frail and have complex care issues may be enrolled in dedicated programs.
Palliative Care
The PISCES (Palliative, Integrative, Supportive, Complementary, Empowering and Spiritual) Transitional Care Unit aims to provide quality care for acute palliative patients. Co-located with Dover Park Hospice, it facilitates the co-management of patients with greater complexity care needs, earlier in their disease trajectory, and facilitate safe returns to the community where possible, with support from home and day hospice services.

Ventilatory Rehabilitation Unit
The specialised Ventilatory Rehabilitation Unit (VRU) looks to provide comprehensive rehabilitation care for recovering patients who require breathing assistance with a machine.
Admission and Ward Information
What to prepare and expect for your stay.
Patients who are being admitted to Integrated Care Hub (ICH) will be informed of the type of step down care they will be receiving. Please have with you the following documents:
- Your NRIC or Birth Certificate (if you are below 15 years old)
- Passport (if you are a foreigner)
- Medical benefit identification documents
- Letter of Guarantee from employer
- Hospitalisation Identity Card
- Civil Service Card, etc
- CPF Membership card, valid P, Q, R, Employment Pass or letter from employer for Work Permit / Employment Pass holders
Wards at TTSH-ICH
Depending on the subsidies you are eligible for, you may have a choice of subsidised or non-subsidised wards.
Before admission, patients or their next of kin can select the room type they prefer.
