Introduction to Geriatric Assessment Services
The Geriatric Assessment clinic provides service to frail elderly persons aged 65 years and above with complex health problems like day to day functional decline, poor basic self-care, reduced mobility, weight loss and decreased appetite.
Patients referred to this clinic are managed by the Geriatric team which comprises Geriatricians, nurse clinicians, physiotherapists, occupational therapists, medical social workers and other allied health workers.
Our Services Include:
- Assessment to evaluate medical functional and social disabilities
- Identify the area of needs for the elderly and family
- Provide interventions aimed to prevent further disabilities
- Provide counseling on patient safety, nutrition, coping with difficult behaviours
- Develop an individualized care plan for the elderly to continue to live at home with loved ones
- Referral to various community services if need arise
